Patient's Guide
The St. Elizabeth General Hospital Information and Admitting Section obtains precise data pertinent to patient information at the time of admission and discharge. With caution and a positive attitude, it offers and enables the prompt, accurate channeling of information to patients, medical professionals, personnel, and other hospital consumers. Regarding patient admission, transfer, discharge, and other department-related services, they collaborate with other departments.
Admission Procedures
Patients with doctor’s admission order sheet will be requested to present all necessary documents required during your stay for immediate processing at the Admitting Section.
In the absence of a Doctor’s admission order sheet, a walk-in patient may be assessed at the Emergency Room and referred to the Admitting Section. Our admitting representative will facilitate your room of choice or assign a room according to you medical need. You will be requested to fill out admission documents namely Patient Sheet Information, Consent forms, and Admission agreement. A nurse will escort you to your room once ready. All patients are given ID band to be worn around the wrist for the entirety of your stay in the hospital.
Patient Information
The patient or his/her responsible party accomplishes and signs the Patient Data Sheet and presents the following at the Admitting Department:

Government issued identification cards

PhilHealth ID

PWD / Senior ID

UMID / Passport / Driver's License

For non-emergency or direct admission under Company or HMO account

HMO or Insurance Card

Letter of Authorization (LOA)

Emergency Cases
All urgent admissions are facilitated and arranged in the Emergency Room. The ER Resident on Duty (ROD) will ask the patient for his/her personal doctor. ER ROD may only assign an attending doctor if the patient does not have a doctor of choice.
Non- emergency Cases
The patient will be requested to present his/her doctor's admitting order slip indicating specific instructions necessary for the patient's case. In the absence of the doctor's admitting order slip, the patient is directed and referred to the Emergency Room (ER) for medical assessment.
Discharge Procedures
Discharge Order

The patient shall not be discharged without the signed order from his / her Attending Doctor(s).

The Nurse on Duty (NOD) :
-
Facilitates the doctor's discharge order;

Advises the patient or his/her responsible party regarding the discharge order; and
-
Coordinates the order with other departments and submits discharge clearance to the Billing Department.

Processing of Final Bill

The Billing Department processes the Final Statement of Account upon receipt of the discharge clearance.

The patient or his/her responsible party will be requested to submit the duly accomplished PhilHealth forms and documents to avail the PhilHealth deduction.

The patient's PWD or Senior Citizen (or any government issued) ID should be presented for discount availment.

Patients under Corporate or Health Care Accounts are required to submit the Letter of Authorization before discharge.
Issuance of Discharge Slip

The Billing Department advises the patient or his/her responsible party to settle the bill at the Cashier.

The Cashier issues the Official receipt and gate pass upon payment.
Discharge Instruction
-
The patient or his/her responsible party presents the gate pass to the Nursing Unit.

The NOD discusses the discharge instructions to the patient or his/her responsible party. The patient will be assisted as he/she leaves the hospital.
